Enhanced Primary Care (EPC), Chronic Disease Management (CDM) & Dept of Veteran Affairs (DVA)

A CDM/ EPC plan is designed for patients with chronic conditions and when physiotherapy treatment will help in the rehabilitation phase. The aim of this service is to improve the health and quality of life of these people with a chronic/complicated or terminal condition.

To receive a Medicare rebate for physio treatment at Central Physio Bayswater a GP must refer you with an EPC plan. If you are eligible you can claim rebates for a maximum of 5 allied health visits (this includes all visits to any allied health practitioner) per calendar year.
